TV Body Count Form

Participants in our TV Body Count Study should submit their counts electronically using our PDF form.
Click on TV Body Count Form to access the PDF form. Please note that it will take a few seconds for the form to open and be ready to accept data entry.
NOTE: You must have the latest version of Adobe Reader installed to be able to use this form. If you do not have Adobe Reader installed, please install it first by going to Adobe Reader Download.
Instructions for using the Body Count Form
The form is designed to record a single TV episode. You may want to print out blank forms and use them to write in your counts while viewing the shows.
- When you are ready to submit your results, open the form on your computer and fill in the required information. Add any notes or description you wish to add in the “Notes / Description” area for each body type.
- Fill out one form per episode watched and then click the “Submit Form” button that you will find in the upper right corner of the PDF.
- Once you click the “Submit Form” button in the previous step, you should see a box appear that says “Send Form” and asks you to enter your email address and Full Name. You must enter this information so we know who submitted the data.
- Click “Send”. Watch your screen to see confirmation messages pop up: “Your response has been queued for submission” and then “Your response was successfully sent.” This means the information on the form was transmitted to Funeralwise.com.
Please note that the information you have entered is not cleared out when the form is submitted. The information entered remains on the form on your computer screen.
You should then save the form to your computer with a new name such as, CSI_1.pdf. You could use the completed form to submit the next episode by changing the counts and episode description or you can start fresh with a blank form. Then you could save the next episode as CSI_2.pdf. You should save a copy of each form submitted in case there is a problem with the transmission. This ensures that you will not lose any of the work you have done. You can delete the files after we have confirmed receipt of all of your submissions and paid you for your work.
